These are like the quick start guide that come with electronics. I used my new posture chart within 2 days because these forms made it easy to get started with a new office service and patient process

I utilize the intake form at the initial visit along with the more explanatory flyer. It primes and educates the patient before the posture analysis. Results are evaluated and discussed at ROF.

Forms are well designed and ready to go. Able to add my practice info plus any other promotion makes it a great marketing and eye catching display. Highly recommend!!

Postural exams are one of the services we offer that patients engage with the most. I plan to use the forms for documentation and the fliers for marketing the screenings.

I was looking for free posture assessment forms and found these and the Posture Pictures book. I did choose the book since the forms come free with it. My practice is diverse and it was helpful to have both a college/high school age person and a boomer age person on the marketing flyers.

I recommend getting the whole book. They give you a download of the forms with it and you can change them and use forever.
